Job Description

Do you enjoy developing and running activities that help support function for greater engagement and health and well-being in adults? As the Program and Activities Manager, you will put those skills to work through overseeing the daily programming in our adult day program for functionally impaired adults 18 years or older, living at home in the community. Our Adult Program is based on a social model of day care, that provides a variety of recreational therapy interventions for the purpose of supporting individual function or buffering the trajectory of chronic/developmental conditions for enrolled adults.

You will develop and manage the recreational therapy calendar for the site, conduct assessments, track and update client progress goals and facilitate contracted services. You will work hands on with our care staff to ensure proper implementation of our programming, while getting to know our clients on an individual level to develop person-centered care plans which includes goals with measurable outcomes.


Duties And Responsibilities

Core Duties and Responsibilities

  • Responsible for the program’s day to day implementation and the comprehensive care and well-being of our program participants.
  • Develop, monitor, and update individualized care plans for each client, including developing client care goals and measurable outcomes for the person-centered objectives.
  • Communicate with staff, family & caregivers for all client care conferences and person-centered planning.
  • Set and conduct family & caregiver conferences
  • Create monthly activity calendars and daily activity schedules, inclusive of community outings and contracted services.
  • Be a resource for direct care staff

Qualifications
  • Must hold one of the following licenses in the state of Utah: CTRS, MTRS, RN CSW, EPPP.
  • Current, valid State of Utah Driver’s License
  • Ability to drive a 12-15 passenger van.
  • Ability to physically assist clients with ambulating, toileting, dressing, lifting 50lbs or more and other needs as assigned.
  • Minimum of 1-year experience working with functionally impaired adults.
  • High level computer literacy
  • Current CPR and first aid certification
  • COVID-Vaccinated
  • Pass a BCI (background screening) and drug test.
