Job Description
Overview:
The mission of Neighborhood House is to
enrich, empower and educate children and adults through quality, affordable day
care and support services. Our Adult Day Services (ADS) program serves aging
and disabled adults with a therapeutic recreational day program. The Program Manager is primarily responsible to develop
and implement quality programming to meet a range of ability levels. The
successful candidate will maintain current Utah
license or certification and manage all recreational therapy for the site, track
and update client progress goals, and support ongoing communication between
clients, caregivers, and the center. Other duties include oversight of program staff performance, compliance with the budget for
program supplies and contracted services, and assistance to the Program
Director in facilitating resolution of staff concerns.

• Current Certified Recreational Therapist (CTRS or MTRS) license in the state of Utah REQUIRED.
• Current, valid State of Utah Driver’s License.
• Ability to train and drive a 12-15 passenger van.
• Demonstrated ability to physically assist clients with ambulating, toileting, dressing, lifting 50 lbs. or more, feeding, and other needs as assigned.
• Communicate effectively in English, with proficiency in Spanish preferred.
• Pass a Utah Department of Human Services BCI (Background Screening), and drug screening.
• Provision of individual work release physical within 30 days of hire.
• Demonstrated desire to care for people, including the frail elderly and/or other adults who are unable to live independently.
• Experience with staff management, regarding program implementation and performance goals.
• Highly motivated, with vision to lead and develop quality, exemplar, program for replicability within the state.
• Solutions oriented, and able to problem solve and/or develop answers with minimal direction, capitalizing on opportunities to learn and add value.
• Highly computer literate with a proficiency of Microsoft Office, and client tracking software database(s).

Duties And Responsibilities
Primary Responsibilities:
• Responsible for the program’s day to day functioning and the comprehensive care and well-being of our program participants.
• Assist the Program Director with developing and reviewing quality assurance procedures.
• Differentiate and implement both high and low functioning programs for approximately 60 clients
• Develop, monitor, and update individualized care plans for each client, including developing client care goals and measurable outcomes for the person-centered objectives.
• Communicate with family & caregivers for Incident & Behavior Reports when necessary.
• Set and conduct family & caregiver conferences
• Provide daily written schedule for staff to carry out calendar activities
• Complete and distribute monthly activity calendar by the 25th of every month
• Be a resource for direct care staff and provide support as needed
• Manage inventory and purchases for program supplies, equipment and activities
• Assist the Program Director in planning, organizing, and implementing client/caregiver events
• Meet with health provider agencies as required for individual clients
•Write and distribute a monthly newsletter to be distributed with the monthly calendar.
• Develop quarterly goals for the program with the ADS Director
